1.1 Creating A Mentor Form

 

1. Navigate to form and click the Create Form button

   
   
   
   
    

2. Give your form a title

   
   
   
   
    

3. If there is a form existing on the platform already with the questions you wish to copy, use the Template drop-down to select the existing scheme form.

   
   
   
   
    

4. Click Create to start adding details and questions to the scheme.
   
   Once created, the editing of any form can be accessed via the edit button listed under the form
